FHCSD's Program for All-inclusive Care for the Elderly (PACE) is designed to help our patients maintain a high standard of independent living. Our program provides a wide range of supportive services including in-home care, meals, medical equipment, transportation, social services, and social activities. We are currently seeking a PACE Activity Coordinator to oversee the Day Center activities at our Oceanside location.

This position is a crucial supervisor role responsible for coordinating and implementing all Day Center activities and social service programs. The PACE Activity Coordinator also creates and maintains daily, weekly, and monthly enrichment activities for our program participants to enjoy. Our participants love music, art, exercise, cooking demonstrations and a wide range of other recreational activities.

Position Details

  • Plan activity programs tailored to the needs, interests, and limitations of frail elderly persons
  • Promote optimum health, independence and self-care
  • Develop custom care plans that meet participants' needs, interests and skills to enhance overall health and wellbeing
  • Strive to increase or maintain social skills and self-esteem
  • Evaluate all PACE participants' suitability for various recreational activities
  • Establish realistic goals and set up a treatment plans utilizing recreation therapy techniques
  • Re-evaluate PACE participants on a regular basis to monitor outcomes
  • Work in collaboration with FHCSD's multidisciplinary team to develop enriching care plans that meet participant's needs, resources, and interests
  • Complete required progress notes and six-month reassessments for PACE participants
  • Supervise and coordinate those assigned to the activities program and make recommendations for evaluations to the Center Manager
  • Post a monthly calendar of events and stablishes an activity volunteer program
  • Assist with in-service training and planning of community events
  • Create and maintain an Activity Program budget that meets the Center Manager's approval
  • Work with community organizations to support and enhance the Activity Program
  • Ensure compliance and remain up to date with orders or directives issues by duly constituted government/regulatory agencies (PACE, CMS, DHCS, etc.)
  • Collect program data needed to prepare and submit required reports
  • Complete all required data entry and documentation in ERH daily

Needed Skills and Abilities

  • Knowledge of psychosocial, behavioral, and family needs of the elderly population
  • Excellent organizational and time management skills
  • Proven ability to work in an interdisciplinary team
  • Ability to maintain a positive work environment
  • Enthusiastic, dependable, resourceful, creative, and flexible
  • Effective oral and written communication skills
  • Strong computer abilities required (Outlook, MS Word, Excel, and other)

Requirements

  • A minimum of 1 year of experience in a social or recreational program within the past five years, preferably in a senior care setting
  • Strong preference for experience in PACE
  • Certified Activity Coordinator preferred, or the equivalent education or job skills needed to perform the position duties
  • Highschool or GED required, associate degree or bachelor's preferred
  • Prefer bilingual (English/Spanish)

  • $19.39 - $27.66 hourly, FT Regular with Full Benefits

19.39 - 27.66

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
