General Job Description

Provides case review to patients within assigned community health program, ensuring the medical program is clinically correct and being followed.

Job Roles

*Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards; in addition, ensures that all client files are kept up to date with RN signature on required documents on a monthly basis.

*Completes Chart Reviews, ensuring medical program requested is being followed.

*Ensures applicable regulatory and department standards are adhered to on an on-going basis (The Joint Commission, OSHA, etc.).

*Performs other duties as assigned.

*Reviews and sign-off on all Case Managed client Care Plans, Initial Assessments, and Comprehensive Assessments.

Education/Certifications/Licenses/Registrations

*1 year of work experience providing human services to high risk, medically underserved, disabled populations, and/or relevant community health populations required.

*Current California Registered Nurse licensure. Nurse Practitioner, Physician Assistant, or Medical Doctor licensure accepted.

*Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.

*Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required.

DRIVER REQUIREMENTS: Age 21; Licensed for a minimum of 3 years; No violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving with 7 years

Experience/Specialized skills (including Language)

*Ability to work effectively with patients from diverse social, cultural and economic groups.

*Ability to work well in both a team-based environment and independently.

*Basic computer literacy ability to comply with department needs and expectations (i.e., electronic medical record documentation, obtaining background information and reports on patients, following up on appointments, etc.).

*Bilingual in English/Spanish preferred.

*Demonstrated ability to be culturally sensitive and respect diversity.

*Excellent organizational skills, attention to detail, time-management skills, and strong motivation to meet deadlines and achieve goals.

In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits or potential bonuses.

$40.00 - $49.95

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
