FHCSD's Program for All-inclusive Care for the Elderly (PACE) is designed to help our patients maintain a high standard of independent living. Our program provides a wide range of supportive services including in-home care, meals, medical equipment, transportation, social services, and social activities. We are currently seeking a Licensed Vocational Nurse (LVN) to provide in office support, and at home care to our PACE participants in Oceanside.

Duties and Responsibilities
- Regularly completes home visits to evaluate home care needs.
- Identify needs for other home care services to ensure the quality of care for each PACE participant.
- Provides accurate and timely documentation consistent with the plan of care.
- Assesses and provides patient and family/caregiver information pertinent to care plan.
- Actively participates in the Quality Management program.
- Ensures the participant-centered plan of care is followed.
- Assist the Home Care Coordinator to schedule staff for home visits and work assignments as needed.
- Coordinates home care delivery and implementation of all home care services.
- Communicates and interacts with PACE participant's caregivers and/or family members.
- Ensures the highest quality of care is provided and follows regulatory requirements, and departmental policies and procedures.
- Acts in the role of liaison between the interdisciplinary team, caregiver representatives and community agencies.
- Provides ongoing education and/or instructions to the PACE participant, family, and/or caregivers about goals and expectations, safety at home, use of equipment and other topics as indicated, and as allowable under the LVN license.
- Participates as a member of the Interdisciplinary Team and assists in the development and implementation of policies and procedures.
- Performs other duties as assigned.
Desired Experience
- 1 - 2 years of experience working with the older adult population preferred.
- Clinical competency with the older adult population is preferred.
- Bilingual English/Spanish preferred.
Requirements:
- Current California LVN License.
- Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.
- Ability and means to travel as needed promptly within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Age 21; Licensed for a minimum of 3 years; No violations and/or accidents within 3 years; No vehicle-related suspensions/reinstatements; No DUI, reckless or felony Driving within 7 years.

Our History
FHCSD is a 501(c)(3) nonprofit organization rich in history and tradition. In 1911, the San Diego Industrial School was built on the site where our first clinic now stands. This school eventually transformed into a settlement house that provided Barrio Logan's immigrant residents with dearly needed social services. When it was announced that the city was closing the building in October 1970, Laura Rodriguez and a group of community members attended a Barrio Logan Community Action Committee meeting where they passionately advocated that the location should continue to function as a community health clinic. When their request was quickly rejected, the group occupied the building in protest.
After eight days of negotiations the City of San Diego and the community leaders reached a resolution. This led to the gifting of the building to the people of Barrio Logan and the establishment of a free clinic. Medical care delivery began two evenings a week, and the health center saw approximately 150 patients each month. This building became our flagship clinic, Logan Heights Family Health Center, and we're proud to honor our heritage by continuing to increase access to quality health care in San Diego. Over the past several decades FHCSD has transformed into one of the 10 largest Federally Qualified Health Centers in the country. We now have over 75 locations, more than 3000 employees, and will continue to grow while we strive to meet the needs of our community.

$30.00 - $36.65If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
